As a United Concordia Companies, Inc. (United Concordia) participating network dentist, you have agreed to provide services to members enrolled under any network access arrangement.

United Concordia currently has active network access arrangements in place with the following plans:

PPO vs HMO – Our contracting remains the same regardless of whether the medical plan is an HMO or PPO. We administer dental benefits under a single fee-for-service model without additional mandates or administrative burdens. The only exception is that members with an HMO medical benefit do not receive dental benefits when receiving care from out of network providers.
